QUESTIONS TO ASK INSURANCE

What mental health benefits does my plan have?

What is my copay?

Do I have a deductible?

What is my co-insurance or copay once my deductible has been met?

Do I need prior authorization before attending a session?

Will my insurance cover my provider/their practice?

Do I have out of network benefits?

INSURANCE WORDS TO UNDERSTAND

COPAY: this is an amount paid by you each visit. This amount is set by the insurance company and your plan.

COINSURANCE: similar to a copay but with coinsurance, your insurance pays a percentage (i.e. insurance pays 80% and you are responsible for 20%).

DEDUCTIBLE: a dollar amount you must pay out of pocket prior to getting support fro insurance. Most deductibles reset at the beginning of the year. After the deductible is met you may have a copay or coinsurance.

PARTICIPATING PROVIDER: provider is contracted with the insurance company and agrees to a set rate set by insurance guidelines.
